- Please bring proof of household income (a current paycheck stub, last year’s income tax return, or other proof of current household income).
- If you have medical insurance through work, Medicaid, or private insurance, be sure to bring in your insurance card.
- Think of any questions or concerns you have so we can address those during your appointment.

ASAC accepts most private insurances and Medicaid (Molina Healthcare of Iowa, Wellpoint Iowa, Iowa Total Care, and Iowa Medicaid).
Our goal is to make our services as accessible as possible, which is why we offer a sliding fee scale for assessments and treatment sessions based on family size and income.
